Co-certifies the terminal prognosis with the attending physician, and leads the interdisciplinary team in the patient’s plan of care.
Works in conjunction with the patient’s primary care physician and the rest of the interdisciplinary team, certifies the terminal prognosis, assesses patient needs, manages symptoms, prescribes treatment and approves the plan of care.
Assesses patient’s and family needs, develops plan of care to meet identified need; coordinates team visits; ensures implementation of approved plan of car; coordinates care with attending physician, primary caregiver, and/or care center staff; provides patient/family teaching as needed; and ensures symptom control and pain management.
Provides comfort measures, reports identified needs to registered nurse, provides direct personal care to patient and emotional support to patient and family.
Addresses the emotional needs of both the paitent and family members, assist with end-of-life preparations including funeral care, provides direct counseling or refers patients and families to appropriate community agencies.
Works in conjuction with patient’s c;ergy, provides spiritual support for patients and family members and develops plan of care to meet identified needs.
